Telehealth services
Telehealth uses electronic communications to allow a healthcare provider and patient in different locations to exchange health information. Services may include questionnaires, secure messages, photographs, records review, audio, or video where appropriate.
Expected benefits
Potential benefits include improved access, convenience, continuity, and more efficient evaluation. These benefits cannot be guaranteed.
Material risks and limitations
- A remote provider may not receive information that would be available during an in-person examination.
- Technology can fail, delay communication, or create privacy risks.
- A provider may determine that in-person examination, testing, or emergency care is required.
- Known and unknown side effects or medication interactions may occur.
Independent clinical judgment
You understand that providers in the LocumTele network are independent professionals responsible for clinical decisions. You may ask questions, decline a proposed treatment, seek a second opinion, or withdraw consent before treatment, subject to services already delivered.
Your responsibilities
You agree to provide accurate information, disclose your physical location, identify all medicines and allergies, follow care instructions, and seek in-person or emergency care when directed. Do not use telehealth while driving or where privacy cannot reasonably be maintained.
Prescriptions and records
A prescription is never guaranteed and will be issued only when a provider determines it is medically appropriate and lawful. The provider and pharmacy may create and retain records according to applicable professional and legal requirements.
Emergency care
Telehealth through Tryinova is not for emergencies. Call 911 or go to an emergency department for chest pain, breathing difficulty, severe allergic reaction, confusion, fainting, suicidal thoughts, or other urgent symptoms.
Consent
By affirmatively accepting this consent during the consultation flow, you acknowledge that you understand the information above, have had an opportunity to ask questions, and consent to receive telehealth services.
